What Happens When I Make a Referral?
Once YFRC staff receive a completed form, we will call the family of the student referred. We will talk with them about their student – the strengths of the student, their talents, skills and interests; and about some of the challenges the student – and the family – may be facing. This is called an “intake.” The intake helps us decide with the family, what the best help for that student may be.
During the intake, we come up with an initial plan and identify goals for our work together, and often decide to bring in help from one or two of our many excellent community resources. The planning phase of the intake is a collaborative, student- and family-driven process.
Following the intake, the referring individual receives a “referral confirmation,” either a note or email from our staff, letting them know that the referral was received and that we have made contact with the family. Because our services are confidential, we will not discuss or share any additional information after confirming receipt and follow-up of the referral.
We check in regularly with our families to discuss progress on the goals they’ve set forth during the intake plan. We adapt and adjust these goals and our work together as needed, to ensure that we are providing the best possible support at all times.
